Position Description
Responsible for ensuring quality and safe delivery of Hospice services through intake triage, daily coordination and follow up of the patient, provider, family and staff's needs.

Essential Functions Statement(s)
• Triages phone calls and offers clinical direction to patients, families and providers as appropriate.
• Coordinates and follows-up on the clinical care needs of the patients, families and providers.
• Receives referrals and obtain intake information for all Hospice Services including notifications of insurance providers for prior authorization and services covered as needed.
• Gathers information necessary to facilitate complete admission process, including provider orders, signed face to face, history and physical, clinic notes, labs, diagnostics, etc.
• Enters patient information into the Electronic Health Record as indicated and generates the referral in the computer software program.
• Assists in the determination of patient qualification for Hospice services.
• Facilitates the organization and/or delivery of supplies and medications, upon referral.
• Coordinates equipment needs with Home Medical Equipment as well as all initial Nursing, Aide and Therapy Services.
• Assists in the coordination of patient transfers to and from facilities.
• Communicates patient, family and provider needs to nursing, social worker and office staff as needed.
• Ensures timely hand off of referral to appropriate staff and adjust schedules accordingly as indicated.
• Participates in patient discharge planning.
• Serves as the liaison between the Hospice department and the providers when the primary RN is unavailable.
• Assists in developing written staff assignments according to staffing patterns, patient needs and Hospice staff competencies.
• Participates in the continuous Performance Improvement program for the department and assists in the chart audit process and Quality Assurance measures.
• Attends required meetings prepared and demonstrates active participation.
• Participates with on-call duty to provide 24hrs/day, 7days/week Nursing coverage to Hospice patients as needed. Assists as necessary in making assessments of patients and families.
• Demonstrates a comprehensive knowledge and implementation of Crossing Rivers Health policies and procedures in conjunction with management.
• Demonstrates knowledge of State Licensure, Federal Certification and Accreditation requirements, as well as State and Federal regulations and agency policy.
Position Requirements
Associate's Degree (two year college or technical school) Required, Field of Study: Nursing
Bachelor's Degree (four year college or technical school) Preferred, Field of Study: Nursing

3-5 years experience as a Registered Nurse preferred
Knowledge of Hospice preferred

Computer Skills:
Basic Microsoft Office

Certifications & Licenses:
Valid Drivers License
RN licensed to practice in the state of Wisconsin and/or Iowa
